Patient Forms
At your initial appointment you can expect to be greeted in a warm and friendly manner. Our receptionist will offer you a beverage and we always have fresh, delicious cookies and fruit. We pride ourselves on running on time. Most patients are roomed within five to ten minutes of their arrival. You will be asked to complete paper work that includes your demographics, medical history and reason for your visit. If we are billing insurance for your visit, we will make a copy of your insurance cards. You should be prepared to pay your co-payment at the time of check in.
New Patients
If you are here for a cosmetic consultation, Dr. Tate will spend time with you discussing the latest treatment techniques and design a plan tailored especially for you. He will write you an all inclusive, hand written quote. The office manager will sit down with you, explain the quote and answer any of your additional questions. If you are here for a medical consultation, Dr. Tate will review your case and discuss the best course of treatment. We make sure that you have ample time to ask questions.
